Special Dietary Concerns



Hastings Food & Nutrition Services does provide meal substitutions for students with special dietary needs when medically necessary. A special diet statement indicating your child's diagnosis and food restrictions must be on file with the Food & Nutrition Services office before your child can be accommodated. This form must contain the clinic affiliation and signature from a licensed physician (MD), physician's assistant (PA), or certified nurse practitioner (CNP).

Note:  It may take several weeks to prepare an individualized meal plan with a completed special diet statement.  Parents/Guardians are responsible to provide meals from home for their student until a completed meal plan is in place.

Special Diet Statement 
 
Lactose Intolerance
Hastings Food & Nutrition Services is required by MN Statute 124D.111 to provide lactose-reduced milk to students who are lactose intolerant and have a requested such a substitution in writing. A doctor's signature is not required. If you would like your child to receive lactose-reduced milk, a Lactose Intolerance Form needs to be on file.

Important Note: This form must be completed every year.
